For the rest of your registration you will need:

° Your SSN

° CHPH Company Code including all dashes (|1131-4P)

° Your MOST RECENT check number including all numbers & dashes (found in the top left corner of your paystub)

° The total current amount for that check before taxes (under the earnings columns)

COMPLETE ALL REQUIRED FIELDS & SELECT REGISTER ME

You will then be prompted to fill out security questions-

When you have finished, click the button to save!
